Policy looks good, I don't see any loopholes. One thing though, when you stated that users who come here solely to use talk pages/forums will not be tolerated, you didn't say what would happen. I'm assuming they'll be warned, saying that they must start contributing, or leave. If they don't leave after being asked, how many warnings do they get? Do they get banned? How long or how many edits must they make to be allowed to continue using the talk/forums? These rules should be laid down, so users will have an idea of how many edits they'll need to make before being allowed to stay. - I hate putting hard numbers on things, because that not only creates issues where we have to keep track of accounting on things like this, but we have to deal with that... Perhaps putting percents on it using Special:Editcount would work better... I'd say if more than 50% of your cumulative edits are on forum & talk pages and you've been active over the course of a week or more (following a week warning) then that would be grounds for a permanent ban. I'm in favor of a warning and a chance to put forward real contributions, but the efforts to get people to turn around can be managed on talk pages. As for punishment documentation... On my userpage, I have information about my character (Hollydapple) and her Clan (RowanClan), like, the leader, the deputy, warriors, etc. etc. I also have userboxes, a list of favorite and least favorite characters--you know. - Different parts of the question have different answers. The Userboxes are acceptable. You do not need to remove those. The information about your fave characters is permissible, too. Though I recommend turning that into an Policy:EssayEssay]] and tucking it away on a subpage. The information about your character and clan, however, is probably best removed. I recommend a transwiki to either Warriors Fanon Wiki or Warriors Fan Fiction Wiki (Whichever fits it best.
